The “Internal Bra” Explained: When I Use GalaFLEX® and When I Don’t

When considering breast surgery, patients often ask me about the “internal bra.” The term has become popular online, especially among women considering a breast lift, breast augmentation, or revision surgery.
The concept is fairly simple. During surgery, I can place an absorbable mesh called GalaFLEX® inside the breast to provide additional support as the tissues recover. Patients commonly refer to it as an “internal bra” because it helps reinforce the breast tissues from within.
It’s a wonderful tool and technique for some women, but it’s not necessary for every woman.
That’s where opinions can differ among surgeons. My approach is to individualize treatment based on your anatomy, tissue quality, surgical goals, and history rather than applying the same solution to everyone.

How Does GalaFLEX Work?
GalaFLEX is an absorbable surgical scaffold designed to provide temporary support to soft tissues. Over time, your body gradually replaces the scaffold with its own collagen and connective tissue.
The goal isn’t to create a permanent internal structure. Instead, the mesh provides temporary reinforcement while encouraging the body to build stronger tissue in the area.
For the right patient, this additional support may help maintain breast shape and position over time.
When I Most Often Recommend an Internal Bra
While GalaFLEX can be used in a variety of situations, I find it most valuable for patients who need additional tissue support. Two of the most common examples include:
- Revision breast surgery. When implants have shifted position, “slid south,” or there have been pocket-related issues, GalaFLEX can provide additional reinforcement where it’s needed most. These patients have already demonstrated that their tissues may need more structural support.
- After significant weight loss. Following major weight loss, the skin and underlying tissues frequently lose elasticity, making it more difficult to maintain breast shape over time. Even after a breast lift, those tissues may not have the same strength they once did. In these situations, GalaFLEX can help reinforce the tissues and support the contour created during surgery.
Who Usually Doesn’t Need an Internal Bra?
This is the part of the discussion that gets overlooked online.
Most routine breast augmentation patients don’t require GalaFLEX. Many breast surgery patients achieve excellent results without additional mesh support, particularly when their tissues already provide a strong foundation for long-term support.
Patients who typically do well without an internal bra include those with:
- Good skin elasticity
- Strong natural tissue support
- Mild to moderate breast sagging
- No history of implant position issues
- No significant weight loss that has affected tissue quality
Can GalaFLEX be used in these situations? Certainly. But does every patient benefit from it? In my opinion, no.
Adding technology or surgical materials simply because they’re available doesn’t always improve an outcome. The key is matching the right tool to the right patient.
The Importance of Individualized Planning
The best outcomes happen when the surgical plan is customized for the patient. Every woman brings a different combination of anatomy, tissue quality, lifestyle factors, and goals.
Some patients clearly benefit from additional internal support. Others already have enough natural support that GalaFLEX adds little value. My goal is to recommend what will improve your result, not what happens to be trending online.
The Bottom Line
The popularity of the “internal bra” has created a lot of interest in GalaFLEX, but it isn’t something I automatically recommend for every patient. Like any surgical tool, its value depends on when and how it’s used.
For patients with implant position issues, revision surgery needs, or tissues that have been significantly affected by weight loss, GalaFLEX can provide meaningful additional support. For others, it may add little benefit because their own tissues already provide the foundation needed for an excellent result.
Breast surgery isn’t one-size-fits-all. My goal is to create a surgical plan that fits your anatomy, your tissue quality, and your long-term goals. Sometimes that includes GalaFLEX. Sometimes it doesn’t.
