How Weight Loss Can Change Your Face and What You Can Do About It
10/16/2026
You’ve lost the weight and reached a goal you’ve worked hard to achieve. Congratulations! But now you may be noticing something you didn’t expect: your face has changed along with your body.
Cheeks may look flatter, the undereye area can appear hollower, and the temples may become noticeably sunken. Skin that once looked firm may begin to sag. Instead of simply looking slimmer, you may feel that your face looks older or more tired even though you feel healthier than you have in years.
These changes are common after significant weight loss, and there are several ways we can address them while still preserving what makes you look like you.
Why Does Weight Loss Affect Your Face?
Facial fat provides fullness and support to the cheeks, temples, under-eye area, and lower face. When you lose a significant amount of weight, some of that volume disappears along with fat elsewhere on your body.
The temples are one area where this is especially noticeable. Loss of fat here creates a hollow or sunken appearance and makes the upper face look more angular.
Skin elasticity also plays a role. Skin that has lost collagen and elasticity doesn’t always adapt completely to your new contours, resulting in loose skin as well as volume loss.
How much your face changes depends on your age, genetics, skin quality, the amount of weight you’ve lost, and where you naturally carry facial fat.
Restoring Lost Facial Volume
When volume loss is the primary concern, we have several options.
- Dermal fillers strategically restore volume to areas such as the cheeks and temples. Treating hollow temples requires particular attention to anatomy and proportion. We want to restore a smooth, natural contour rather than simply fill the area.
- AlloClae offers another approach to soft-tissue volume restoration. This structural adipose filler uses purified donor fat tissue to add volume and improve contour without requiring fat to be harvested from your own body.
- Fat transfer uses your own fat, harvested from another area of the body and carefully placed where additional volume is needed. It’s especially useful when we’re restoring volume to multiple areas of the face.
Which option is appropriate depends on how much volume you’ve lost, where you’ve lost it, and your facial anatomy.
The right treatment after weight loss isn’t about putting back everything you’ve lost. It’s about restoring balance while preserving the features that make you look like you.
It’s Not Just About Adding Volume
Restoring lost volume isn’t always necessary. For some patients, loose skin is the primary concern, while others have a combination of volume loss and laxity.
That’s why adding filler alone isn’t always the answer. We need to identify what’s causing the changes and choose treatments that restore natural facial proportions without creating unnecessary fullness.
When Loose Skin Is the Bigger Concern
When significant weight loss leaves loose skin along the cheeks, jawline, or neck, a facelift, neck lift, or combination of procedures can reposition deeper facial tissues, remove excess skin, and improve definition. Surgery can also be combined with fat transfer when we need to address both laxity and volume loss.
My goal is a refreshed, balanced appearance rather than one that looks tight or overdone.
When Should You Consider Treatment?
If you’re still actively losing weight, I recommend waiting before making decisions about facial surgery. Continued weight loss can lead to additional changes.
Once your weight has stabilized, we have a clearer picture of your facial contours and can determine which concerns remain. Nonsurgical treatments may be appropriate earlier based on your individual circumstances.
A Plan Designed for You
During your consultation, we’ll evaluate your facial volume, skin quality, laxity, anatomy, and overall proportions. From there, we can determine whether dermal fillers, AlloClae, fat transfer, facial surgery, or a combination of treatments makes the most sense for you.
