Marionette Lines and Nasolabial Folds: Differences and Treatment Options

Aging brings visible changes to your skin, especially more visible or deepening wrinkles and folds. Two of the most common complaints from patients are the marionette lines and nasolabial folds. While both affect the lower face, they have different characteristics and develop due to distinct factors. Understanding the difference between the two can help you decide on the right treatment. 

  • Marionette lines are the vertical wrinkles that extend from the corners of your mouth down toward the chin, resembling the mouth of a marionette puppet. These lines can make you look displeased, sad, or angry even when your face is at rest.
  • Nasolabial folds, also known as “smile lines” or “laugh lines,” are the deep creases that run from the sides of your nose down to the corners of your mouth. These folds can become more pronounced as you age, giving your face a tired or aged appearance.

Causes of marionette lines and nasolabial folds

Marionette lines and nasolabial folds have distinct causes:

Marionette lines:

  • Gravity: Over time, gravity pulls facial structures like skin & fat pads downward, contributing to the formation of these lines.
  • Laxity & Loss of collagen: As we age, skin loses elasticity, leading to the appearance of deeper creases.
  • Facial expressions: Repetitive movements of the mouth, such as smiling or frowning, can deepen these lines over time.

Nasolabial folds:

  • Genetics: Some people are more predisposed to developing prominent nasolabial folds because of facial anatomy.
  • Volume loss in the upper face: As collagen and fat diminish with age, skin begins to sag, leading to deeper nasolabial folds.

Both marionette lines & nasolabial folds:

  • Weight changes: losing weight (especially rapid weight loss) can affect the fat pads in the face, causing them to shrink and shift, while providing less support for the skin above. This can cause some areas to look more gaunt while deepening lines. 
  • Sun exposure: Over time, UV rays can damage your skin’s underlying structure, worsening skin quality exacerbates these folds.
  • Smoking: Smoking accelerates skin aging, contributing to worsening lines and wrinkles.

Treatment options

These options are effective for both marionette lines and nasolabial folds:

  • Botox: While not as commonly used, neuromodulators (e.g., Botox, Daxxify, Dysport) may be effective for some patients to relax the muscles around the mouth, preventing further deepening of the lines and folds.
  • Dermal Fillers: Injectable fillers, such as hyaluronic acid-based fillers (e.g., Juvéderm or Restylane), can plump the skin and smooth out marionette lines. Results typically last 6–12 months. Fillers are one of the most common treatments for nasolabial folds. Hyaluronic acid fillers are injected into the area to restore volume and smooth out the folds. Results typically last 6–18 months.
  • Chemical Peels: These peels help exfoliate the top layer of the skin, promoting collagen growth and reducing the appearance of fine lines and folds, while brightening skin tone.
  • Radiofrequency (RF): Varying degrees of radiofrequency — ranging from Morpheus8 to FaceTite — are effective at stimulating collagen production and tightening the skin, thereby minimizing wrinkles and folds.
  • Fat Transfer: Fat can be harvested from other parts of your body and injected into the nasolabial folds to smooth the area. This offers a more permanent solution compared to temporary fillers. It is less effective for marionette lines.
  • Facelift: For more severe cases, a surgical facelift may be the most effective option, providing longer-lasting results.
